Registering with a GP practice
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill council tax bill or something similar. Driving Licences are useful as a second form of ID.
The official practice area is G31 & G40. Patients from G32 are accepted by agreement.
You will need to complete a registration form (GMS1) which you can collect from reception and also a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients can be offered a health check with our nurse to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of completed registration. Please contact reception for further information.
When you register with us, we send out confirmation of your registration to your home address, therefore we would require proof of your current address to complete reqistration. This also helps to match to your existing NHS records.
Registering as a Temporary Resident
You can only register with a GP practice if you’re staying in the area for more than 3 months. If you’re not, you may still be treated but as a Temporary Resident.

Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We can set this up for you if you are both registered with us.
All we need is written consent from the patient, or if it is a child 16 and over, again written consent for what they wish full for or partial access.
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
